Filling in the form online will save you time before Mike visits you to discuss how he can help.  Any information is treated in the strictest confidence.

Please be as honest as possible (he will not be judging you, but trying to help you and your pooch). The more information, the better chance there is of Mike being able to help.

When the form is complete, just hit the button at the bottom and Mike will be in touch, promptly.
